If entering grades EC, Pre-K, Kindergarten, 2nd or 6th grade must also meet those requirements, in addition to:

___ Certificate of Child Health Exam within 1 year prior to start of school - After Aug 15, 2023

___ Required Immunizations (part of Child Health Exam)
___ Dental Exam completed after November 15, 2023
___ Eye Exam completed after October 15, 2023

If transferring from another school please have your former school send over their medical records for the items listed above.
